Flowers, Remnants and fragments

The terms get mixed up because guides use them differently:

A Remnant, or fragment, is the single pickup you find in the world.
A Memory Flower is the complete set. It takes between one and six Remnants.
Blue flowers give glimpses into the past when you find them.

According to the PlayStation Blog, finding all the flowers for a given story raises Soul's health, so a half-finished set may not give you the reward yet.

Game8 says its list is still being updated. The two guides also group things differently: PowerPyx lists one pickup in Weeping Creek Valley and Game8 has no group for that region. Neither total is confirmed, so don't worry if your count is off by one.

Is the Memory Flower skill tree bug real?

Possibly, but it isn't confirmed. One Steam player reported that their health went up after completing flowers while the skill tree still counted zero. Replies suggested different causes, including that only large complete flowers count, or that the count updates later. We can't tell which is right. If it happens to you, finish another flower before assuming your save is broken, and keep a backup save.

Tips for collecting them

Get them before bosses where you can. Health upgrades make hard fights easier, and Gamersky's route guide suggests finishing a region's set before its final boss.
Check what becomes hostile. PowerPyx reports that some Mandate Encampment flowers drop from NPCs who turn hostile, and that this can be buggy. See the Mandate Encampment guide.
